چکیده

A 27-year-old Chinese male presented with a 2-year history of headache and a right temporal field defect. A magnetic resonance imaging (MRI) scan revealed a 5cm tumour in the sphenoid with bilateral cavernous sinus extension and posterior extension to the clivus (Figure 1A). This was reported as an erosive skull base tumour and he was referred to neurosurgery. His serum prolactin was measured at 283 909mU/l (normal range 45–375mU/l), confirming a giant prolactinoma. After a single dose of 250mcg cabergoline, there was an improvement in his visual field, tumour reduction and fall in prolactin to 57 539mU/l (Figure 1B). An increased dose of 250mcg twice weekly resulted in further tumour shrinkage, a fall in prolactin and normalization of his visual fields (Figure 1C and D). This case highlights the importance of measurement of serum prolactin in the evaluation of potential skull base tumours and the rapid shrinkage of giant prolactinomas with primary medical therapy obviating the need for surgery. Impressive shrinkage of a giant prolactinoma treated with cabergoline in a prepubescent girl. What now?

Giant prolactinomas are extremely rare in the pediatric population. We describe the case of a giant prolactinoma in a girl aged 14 years and 9 months old presented with delayed puberty. Medical treatment with dopamine agonist cabergoline resulted in a rapid normalization of prolactine levels and an impressive shrinkage and liquefaction of the mass as illustrated in serial MRIs. The therapeutic ...

Delayed presentation of late-onset cerebrospinal fluid rhinorrhoea following dopamine agonist therapy for giant prolactinoma

UNLABELLED Therapeutic shrinkage of prolactinomas with dopamine agonists achieves clinical benefit but can expose fistulae that have arisen as a result of bony erosion of the sella floor and anterior skull base by the invasive tumour, resulting in the potential development of cerebrospinal fluid (CSF) rhinorrhoea, meningitis, and rarely pneumocephalus. Onset of symptoms is typically within 4 mo...

Rapid improvement in visual loss with cabergoline treatment in a giant prolactinoma case: 5 years survey.

Giant prolactinoma is a rare subset of macroadenomas. Limited studies demonstrated which therapy could be successfully used in the first-line therapy of giant prolactinoma. We presented a case with a 54 × 40 × 40 mm pituitary adenoma and optic chiasmatic compression with left sphenoid sinus invasion. The tumor caused a loss of visual field of the right side. Cabergoline treatment was started wi...

Pneumocephalus during cabergoline treatment of an invasive macroprolactinoma.

A 43-year-old male trisomic patient was referred for acute left headache and progressive homolateral mydriasis and ptosis. Computed tomography and T2-magnetic resonance imaging demonstrated a 4 cm invasive pituitary adenoma (Panels A and C, respectively). Blood analyses revealed hyperprolactinemia (5460 ng/ml, normal values < 15 ng/ml) and severe gonadotrophic insuffi ciency. Treatment was clas...

Brain and optic chiasmal herniation following cabergoline treatment for a giant prolactinoma: wait or intervene?

OBJECTIVE Dopamine agonists (DA) are the treatment of choice in patients with macroprolactinomas. Brain and optic chiasm herniation are unusual complications following treatment with DA. REPORT We present a case of a giant prolactinoma complicated by visual deterioration following cabergoline treatment. A 42-year-old man was admitted with seizures, right visual loss and visual defect in the u...
